“Drugs don't work in patients who don't take them"
US Surgeon General, C.Everett Koop
The Problem
Medication non-adherence is a major driver of preventable illness, hospitalizations, and healthcare costs. It occurs in two key forms:
Misuse and Abuse of Prescription Medications
In 2020, millions in the U.S. misused prescription drugs:
• 9.3 million misused opioids
• 6.2 million misused sedatives/tranquilizers
• 5.1 million misused stimulants
This results in an estimated $72.5 billion annually in healthcare costs, with opioid abusers incurring 8.7 times higher costs than non-abusers.
Underuse in Chronic Disease Treatment
Up to 50% of patients don't take medications as prescribed, contributing to:
• 10% of hospitalizations
• $100 billion in excess healthcare costs annually
Impact on Clinical Trials
Non-adherence also undermines drug development. Fewer than 50% of trial participants fully adhere to medication regimens, increasing costs and contributing to the 80% failure rate of clinical trials.
